A Teleport flank needs a ward behind or beside the enemy team, in a spot they cannot see, placed before the fight starts. Your team places that ward early, keeps the enemy from clearing it, and the Teleport player lands on it once the fight begins. To stop flanks, clear the wards on your own sides and watch for the enemy Teleport player going missing.

What a flank Teleport is
A flank Teleport lands one player behind or beside the enemy team during a fight, often the top laner. It works because the enemy is facing your team in front of them and does not expect damage or crowd control from behind.
Later in the game Teleport can land on allied wards and minions, not only towers. That is what makes the ward on the flank so valuable.
Place the ward before the fight
The ward has to be there before the fight starts, in a spot the enemy is unlikely to sweep: a brush or corner behind where they will stand around the objective. The support or jungler often places it as part of the objective set-up.
Land at the right moment
Teleport takes a few seconds to channel, and the enemy can see it start. Teleporting too early shows your plan and lets them turn on the player who lands. Teleporting too late means the fight is already over.
Start the channel so you land just as the fight begins, when the enemy is already committed to your team in front of them.
Stop the enemy flank
If the enemy top laner has Teleport and is missing from the map, expect a flank. Sweep the brushes and corners on your sides before the fight, and keep someone near your back line.
Common Teleport flank mistakes
Placing the flank ward too late, after the fight has already started.
Putting the ward where the enemy will sweep, so it is gone when you need it.
Fighting without checking where the enemy Teleport player is.
